Majority of Americans concerned about medical errors

Almost a third of Americans reported experiencing (or a close friend or family member) a medical error, according to a survey conducted by Wolters Kluwer Health. Such errors include wrong treatment, wrong dosage, wrong medication, etc. Completed in July 2012, this survey consisted of telephone interviews with U.S. consumers. Over 70% of them reported being very concerned or somewhat concerned about medical mistakes, and more than 20% reported having been misdiagnosed by a clinician.

Also revealed in the survey, Americans are taking additional steps to help minimize mistakes. 66% had said to have done their own research to validate a diagnosis or treatment plan. Over half (56%) sought a second opinion. Some other steps taken to prevent medical errors included delaying procedures until a day when a clinician might be more rested or focused, writing information down for the doctor or nurse, and, surprisingly, asking a clinician or nurse to wash their hands (18% of respondents admitted having to ask). Women are more likely than men to do research and get a second opinion. The survey also showed that consumers feel that the biggest causes of medical mistakes are miscommunication among hospital staff (35%), clinicians or nurses being in a hurry (26%), doctors or nurses being fatigued (14%), and staffing shortages in hospitals (12%).

On a good note, the survey respondents showed confidence in technology being able to help reduce medical errors. 68% of the survey agreed that technology has had a positive impact in reducing the chance for medical mistakes.

Getting a Second Opinion…From your Smartphone?

According to a new Pew Research Internet Project study, over half of U.S. smartphone users consult their phone before a doctor for health advice.
52% of people use their smartphone to gather medical information, compared with the 6% of non-smartphone owners.
3,014 people were surveyed and 53% classified their device as a smartphone – Pew’s report indicated them as an Android, iPhone, BlackBerry, or Windows Phone; while 40% reported owning a regular cell phone. Out of all the participants, 1 in 3 browse on their device for health details, which is more then double from the 17% two years ago.
Among all phone owners, Latinos, African Americans, college graduates, and those between 18 and 49 years old are more likely to dig into their cell to look up advice, according to Pew.
9% of smartphone owners also receive text updates and alerts about medical issues, even after doing their own research. They have apps for that. Pew reported that exercise, diet, and weight applications are most popular, though apps that track menstrual cycles, blood pressure, pregnancy, blood sugar or diabetes, and medication are also likely download candidates. The WebMD app was cited by 4 percent of people.
Other types of health apps also exsist according to Pew, some include “brain trainer,” “medical handbook,” “triage,” “anatomy,” “medical diagnosis app,” and “apps for monitoring fluids for kidney stones.”

Firms Seek Medical Second Opinions

Reviews seen as tool to reduce costs of complex cases

Second-opinion medical services can be an effective benefits cost-containment tool for mid-market employers as treatment costs for complex cases such as cancer and back injuries are soaring.
Misdiagnoses are discovered up to 20% of medical cases and treatment changes are recommended in more than half of them, according to recent data collected by three major second-opinion medical service providers.
Over $150,000 of savings was a result due to treatment changes to a 61-year-old patient who had undergone spinal fusion surgery according to Dr. Jonathan Schaffer, managing director of the Cleveland Clinic’s MyConsult Online Medical Second Opinion service.
“Her doctor wanted to extend the fusion further up her spine, but we found that the previous fusions didn’t create the appropriate spinal curvature,” he said.
One reason for the high frequency of misdiagnoses and incorrect treatment plans is that many physicians do not regularly address such complex cases.
“According to a recent analysis of 330 oncology cases by Partners, 12% of the cases reviewed resulted in changes in diagnosis, while 90% resulted in either a new plan or a significant change in prior treatment plan.” said by Dr. Joseph C. Kvedar, director of the Center for Connected Health at Partners HealthCare System Inc. in Boston.
“I don’t think it’s a knowledge deficit of local providers, but those doctors aren’t treating a particular disease over and over” like the specialists at Partners are, Dr. Kvedar said.
Best Doctors was founded 22 years ago by several Boston physicians to serve mostly large, self-funded employers, said Evan Falchuk, president and chief strategy officer. But as the cost of health care rises for employers of all sizes, he is seeing more interest among smaller and midsize employers who pay $2 to $3 per employee per month for Best Doctors’ second-opinion service.
“Everybody wants to have a healthier workforce. I can do everything I can to help my employees stop smoking and lose weight, but if they get sick through no fault of their own, they need help to navigate the system,” he said.
In the case of a patient who was not responding to lung cancer treatment, for example, “Best Doctors reviewed medical records and found that the patient had previously been treated for thyroid cancer. The evaluation discovered that the thyroid cancer had metastasized in his lungs, so it wasn’t lung cancer. This is typical. Twenty percent of people end up with the wrong diagnosis in general,” Mr. Falchuk said.
The service from MyConsult also began taking on large, self-insured employers when it launched in 2002. According to Dr. Schaffer, a lot of smaller employers are also becoming more interested, with as few as 500 employees.

Kidney transplant thrown in trash by mistake

The University of Toledo Medical Center has suspended its transplant program after a nurse threw out a kidney.

University of Toledo Medical Center – A live donor was kind enough to give away his kidney. The next thing he knew, they misplaced it and ended up at the bottom of a trash can. According to ABC News, doctors attempted to revive the kidney when they discovered the error. The attempt was unsuccessful because the organ had been damaged too much. Now the hospital’s transplant program has been suspended.

ABC News and other reporters have pinned the mistake on a nurse for the misplaced kidney. According to the Toledo Blade, another nurse told them the surgeon should have been held accountable.”Where in the recovery, flushing, and prep of a kidney, and transport to the other [operating] room is a nurse involved? Whoever was the primary surgeon should be held accountable,” Donna Luebke, a registered nurse told the Blade. Two nurses have been suspended for this mishap, but the surgeon Dr. Michael Rees has not faced any discipline.

The Toledo Blade also discovered a 2009 lawsuit both the surgeon and one of the nurses. In this lawsuit, filed by surgical technician Kelly Haas, claims that Dr. Rees kicked her in the back during surgery. Dr. Rees was seemingly upset because Haas was leaving the operating room so a different nurse could take her place for the day, but Dr. Rees didn’t think the new nurse was qualified, noted by the Blade.

University of Tennessee Alumnus Inducted as a Fellow in the American College of Radiology

Reston, VA — David C. Chaffin, MD, has been inducted as a Fellow in the American College of Radiology (ACR). The induction took place at a formal convocation ceremony during the recent 89th ACR Annual Meeting and Chapter Leadership conference April 21-25, 2012, in Washington, DC.

Chaffin is currently a diagnostic radiologist for USARAD.com. He is a member of the ACR, the Radiological Society of North America and the Society of Breast Imaging. Chaffin received his medical degree from the University of Tennessee College of Medicine.

One of the highest honors the ACR can bestow on a radiologist, radiation oncologist or medical physicist is recognition as a fellow of the American College of Radiology. ACR Fellows demonstrate a history of service to the College, organized radiology, teaching, or research. Approximately 10 percent of ACR members achieve this distinction.

As CT scans become more common for children, concerns about radiation grow

The use of CT scanning has surged among the young, which has parents and doctors raising concerns, although the risk is relatively small. Around 7 million scans are performed annually in children in the U.S., a figure which has been raising 10% a year, according to the Alliance for Radiation Safety in Pediatric Imaging, a consortium of medical organizations.

According to the National Cancer Institute, many children get more than one scan. About a third who have undergone CT scans have had at least three.

“There have been significant increases. What we’ve seen in adults has been mirrored in kids,” said Donald Frush, chairman of the American College of Radiology’s pediatric radiology commission. “We do much more by CT.” He also noted, “CT is a very, very helpful tool. It is lifesaving.”

Frush and other doctors said there’s a good reason for that: The scans produce detailed images that can help them diagnose and monitor problems.

Sometimes though, doctors said, it is overused as parents push for an extra measure of assurance that a child is all right and as physicians try to guard against getting sued.

An alliance campaign developed five years ago called Image Gently, has pledged to reduce unnecessary CTs with 15,798 signing. The campaign promotes performing scans only when the medical benefit is clear. According to a recent study, numbers suggest CTs performed in children have been starting to drop in some places.

A growing number of doctors are becoming more skilled in knowing when and how to use CT on children. Frush reiterated that CTs are sometimes the best course of action, especially when a child’s condition is serious or difficult to diagnose.
